Thank you for choosing Kristen Lilla for your therapeutic services. Kristen Lilla provides therapy services related to sexuality, desire, and relationship issues. Prior to your first session, you will need to complete the following paperwork: Agreement of Services, Client Information, and review of a Disclosure statement. Filling out your personal information prior to your first session will allow Kristen Lilla the opportunity to review the client information provided and your goals for sex therapy. It will also give you a chance to make a list of any questions you have about sex therapy and consider what your goals for sex therapy are. Filling out these documents prior to your first session will save valuable time for both you and Kristen. It allows you to immediately start working towards your goals during your first therapy appointment. If you do not understand any of the paperwork, or do not feel comfortable answering any of the questions in the Client Information, Kristen would be happy to assist you during your initial session and can answer any questions you may have.
If you are unable to complete the paperwork prior to your first sex therapy appointment, you will need to arrive early to your first session, or plan on spending about the first ten minutes of your session doing paperwork. Kristen will do her best to accommodate you so that filling out your paperwork does not cut into your session time. However, due to scheduling, this is not always possible. Hence, it is encouraged to fill out all paperwork prior to arriving to your first appointment. Please note that all paperwork completed will be kept confidential.
